OTA License Requirements

Before applying for licensure, please familiarize yourself with the general licensing policies.

 

In order to be eligible for Connecticut licensure, an applicant must meet the following requirements:

 

Hold an associate's degree in occupational therapy assisting  from a program accredited by the American Occupational Therapy Association (AOTA);

 

Please note that recent graduates of occupational therapy assistant schools are eligible for a temporary permit to practice under the supervision of a Connecticut licensed occupational therapist. 

 

Satisfactorily completed at least eight (8) weeks of supervised field work experience at a recognized educational institution or a training program approved by the educational institution where the academic requirements were met; and

 

Successfully completed the National Board for Certification in Occupational Therapy (NBCOT) certification examination.

 

Documentation Requirements

 

Applicants must arrange for the submission of the following documentation directly to this office from the is

 

Official transcript of occupational therapy education verifying degree awarded and date of graduation;

 

Official report of NBCOT examination scores:

 

The Department obtains examination results electronically for applicants who completed the examination within the past calendar year for purposes of obtaining licensure in Connecticut.  Applicants will be notified if their involvement with this documentation requirement is necessary;

 

Applicants applying for Connecticut licensure who are licensed in other states shall arrange for an official NBCOT Certification Verification Letter to be submitted directly to this office;

 

If applicable, official verification sent directly from each state licensing authority where a license or certification is or has ever been held.  Most jurisdictions charge a fee for this service.  Please contact the jurisdiction for fee information;

 

A completed application and fee in the amount of $200.00.  Applications are only accepted online.  Please select this link to apply online.
