The Federal Government has reopened COVIDtests.gov. Every household can order four over-the-counter COVID-19 tests for free. These tests are intended for use throughout the 2024 holiday season to detect currently circulating COVID-19 variants. Please visit COVIDtests.gov for your free test.

Certification Requirements

Before applying for licensure, please familiarize yourself with the general licensing policies.

 

An applicant for a community health worker may qualify for certification provided the applicant: 

Is trained and educated as a community health worker by an organization approved by the Community Health Worker Advisory Body;

 

Is at least sixteen years of age;

 

Submits a professional reference from an employer with direct knowledge of the applicant's experience as a community health worker;

 

Submits a reference from a member of the community with direct knowledge of the applicant's experience as a community health worker, and

 

Has a minimum of one thousand (1,000) hours of experience working as a community health worker during the three years prior to the date of such application;

 

OR

 

Has a minimum of two thousand (2,000) hours of paid or unpaid experience as a community health worker;

 

Submits a professional reference from an employer with direct knowledge of the applicant's experience as a community health worker

 

Submits a reference from a member of the community with direct knowledge of the applicant's experience as a community health worker.

 

Application instructions

 

As part of the application, applicants will be prompted to upload the following documents:

 

  • A recent photo of the applicant;
  • Verification of experience form documenting the appropriate number of hours of paid or unpaid experience;
  • If applicable, a letter from an approved training provider documenting completion of the required training;
  • A letter of reference from an employer with direct knowledge of the applicant's experience;
  • A professional reference letter from a member of the community with direct knowledge of the applicant's experience as a community health worker.
