Document a vaccine return (PDF) in CT WiZ for any full, unopened doses that need to be returned to the manufacturer.
Reasons vaccines may be returned include:
- Vaccine has expired
- Vaccine is no longer viable due to a temperature excursion
The Main Contact/Shipping Contact will receive a UPS Quantum Return Label from a 'no reply e-mail' to return doses to the manufacturer once a vaccine return is approved in CT WiZ and submitted to the manufacturer.
If your non-viable vaccine meets any of the following criteria, document the wastage in CT WiZ and do not complete a return:
- Broken vials/syringes
- Partial vials of vaccine
- Vaccine that has been drawn but not administered
Wasted vaccines should be properly disposed of and adjusted out of CT WiZ inventory.
Reasons vaccines may need to be adjusted (PDF) out of your inventory on-hand include:
- Borrowing vaccines
- Opted out patients
- Inventory correction (requires an explanation for why you are completing a correction)
All vaccines returned, wasted, and adjusted are reviewed by the Department of Public Health Vaccine Coordinator. For more information on different adjustment and wastage reasons, please refer to our Inventory Adjustment Reasons (PDF) table.
