Who receives the final report?
Once screening results are finalized by the Connecticut Newborn Screening Program at the State Public Health Laboratory, a copy is faxed to:
- The hospital that submitted the specimen
- The primary care provider listed in the Newborn Screening System
Hospitals are required to enter the newborn’s primary care provider information before submitting the sample.
Why is my facility not receiving results?
Ensure that the Connecticut Newborn Screening Program has the correct fax information for your practice/facility.
It is the provider’s responsibility to maintain a fax machine capable of receiving newborn screening final reports and ensuring they are included in the medical record. If faxing is not feasible, you may provide a secure email address for report delivery.
Troubleshooting Missed Faxes
If you’re unsure whether your facility is receiving final reports, please ensure your fax machine:
- Has sufficient memory and paper to receive multiple page
- Is not configured to reject or pause after a certain number of faxes
- Is monitored regularly to avoid missed transmissions
- Does not automatically sort faxes into a spam folder
Note: Reports are sent under the newborn’s birth name (e.g., “Baby Girl/Boy [Mother’s Last Name]”). If your system auto-sorts faxes into electronic health records, manual review may be needed.
I verified my fax machine is working correctly and that we have not already received the final report, what now?
If you still have not received the report after verifying the above:
Fax a request to (860) 730-8385 including:

Newborn’s name

Date of birth
-
Mother’s name
-
Hospital of birth
-
Your facility’s fax number
-
Or call (860) 920-6628 with the same information to request the report

For Parents
I am a parent; how can I get a copy of my child’s newborn screening results?
Due to privacy laws, we are unable to provide protected health information to parents over the phone. Parents may request a copy of their child’s newborn screening results by contacting the pediatrician or primary care provider.
You may also request a copy of the report by completing our Authorization to Disclose Newborn Screening Protected Health Information form.
You may email the completed form including a copy of photo identification to dph.nbstracking@ct.gov or mail it to QA Manager, Dr. Katherine A. Kelly State Public Health Laboratory, 395 West Street, Rocky Hill, CT 06067.
For Student Athletes
I am a student athlete looking for a copy of my newborn screening results for NCAA requirements, can I get a copy?
Why?
Prior to 2013, records were only retained for 5 years per CT State Statutes. Records are now retained longer to support NCAA requirements for future student athletes.
