All organizations that received a 2025 rate schedule from the Office of Emergency Medical Services must complete a 2026 rate application and spreadsheet. Rate applications are due no later than August 31, 2025.
Providers that have not yet begun to impose approved charges should complete the “Non-charging” Service Certification Statement for 2026.
