- Homebound Vaccination Intake Form (for individuals who are homebound and cannot attend a community clinic)
- Submit a Secure Online Submission Form (if your or your minor child's immunization record is missing immunizations or was not found in the CT WiZ Public Portal)
- Submit a Helpdesk Request (for Providers, School Nurses, LHDs, or EHR Vendors who need support from the Immunization Program)
