EEIP Reportable Disease Follow-up Guidance for State and Local Health Department Employees
The Connecticut Reportable Infectious Diseases Reference Manual (the manual) describes infectious disease investigation and follow-up recommendations of the Connecticut Department of Public Health (DPH) Epidemiology and Emerging Infections Program (EEIP) and outlines the responsibilities of DPH and local health departments (LHD). The manual provides information to help public health practitioners in Connecticut implement nationally recognized disease prevention and control measures. Primary users include Connecticut local health directors, public health nurses, epidemiologists, sanitarians, other LHD personnel, and DPH employees.Please keep in mind that the Reportable Diseases List and the list of Reportable Laboratory Findings are reviewed annually and may be updated. Scope of the manual
The focus of the manual is on reportable infectious diseases covered by DPH EEIP, particularly foodborne and enteric diseases. The manual does not address vector-borne diseases (mosquito-borne and tick-borne diseases), for which DPH staff conduct follow-up, or possible agents of bioterrorism. The manual also does not address diseases covered by DPH Healthcare-Associated Infections Program, Immunizations Program, Respiratory Viral Disease Program, STD/HIV/Hepatitis Programs, or Tuberculosis Program, which have their own disease-specific recommendations. - Control of Communicable Diseases Manual, 21st Edition (American Public Health Association, David L. Heymann, MD, Editor), or most recent edition;
- Red Book: 2021-2024 Report of the Committee on Infectious Diseases, 32nd Edition (American Academy of Pediatrics, David W. Kimerlin, MD, FAAP, Editor), or most recent edition.
Much of the disease-specific information in the manual is derived from these two resources, and from materials available on the CDC website including the Nationally Notifiable Diseases Case Definitions.
Forms
Reportable disease forms are found on the DPH Forms page. The primary disease reporting form for providers is the Reportable Disease Confidential Case Report Form PD-23. Laboratories use the Reportable Laboratory Findings Form OL-15C to report findings. Disease specific report forms and case interview forms are available for some diseases.
