What tax forms should we submit with our CAEF application?

 

Applicants must submit either of these IRS forms to substantiate their organization's total donor contributions:

The IRS form must be complete, signed, and filed with the Internal Revenue Service (IRS) before you apply. If the IRS form has not been filed, it cannot be used to qualify for the CT Arts Endowment Fund.


We do not accept:


  • Draft IRS forms
  • IRS forms not filed with the Internal Revenue Service (IRS)
  • IRS forms 990-N (e-postcards) and 990-PF
  • The same two IRS forms submitted the previous year if you are a returning grantee.
