In order to search for a profile, please select the following link and enter the name or license number in the appropriate fields. 

 

If you are unsure of the spelling, use only the first few letters of the licensee's last name and select 'Search'. 

 

At the next screen, select the 'Detail' link next to the licensee's name;

 

At the next screen, select the 'View Profile' button at the bottom of the page.  The physician profile information will then display.

 

Licensees who wish to update their profile should select this link for more information.
