Chiropractic Licensure Reinstatement of a Lapsed License

Before applying for licensure, please familiarize yourself with the general licensing policies.

 

An applicant for reinstatement of a Connecticut license that has lapsed due to nonrenewal shall arrange for the submission of the following documentation directly from the source:

 

A completed application form and fee of $565.00 in the form of a bank check or money order payable to, Treasurer, State of Connecticut”;

 

A brief written summary of the applicant's professional activities since graduation from chiropractic college, to include work history (positions, dates, duties), any lapses in active practice as a chiropractic, and if not currently practicing as a chiropractor, an explanation of the applicant's plans for employment and description of how the applicant intends to refresh his or her skills;

 

A letter submitted directly to this office from the appropriate authority verifying the applicant's most recent clinical experience confirming employment, including dates and evaluation of clinical performance.  If in private practice, a letter from a practitioner with whom the applicant has had a referral relationship;

 

Verification of all licenses held, current or expired, forwarded directly from the appropriate authority in any U.S. state, territory in which the applicant is or has ever been licensed as a chiropractor.  Most jurisdictions charge a fee for completion of the verification form.   Please contact the jurisdiction for fee information.  Please select this link for the required form.  

 

Please note that all reinstatement applications from chiropractors who have been out of the active, clinical practice for longer than one (1) year are reviewed by the State Board of Chiropractic Examiners.  The Board meets quarterly.

 

Applicants who have been out of active clinical practice for two (2) years or less must be in compliance with the continuing education requirements.

 

In most instances, applicants who have been out of the active clinical practice for more than two (2) years are required to complete the Special Purpose Examination in Chiropractic (SPEC) administered by the National Board of Chiropractic Examiners.
