How to Obtain Important Discharge Documents
The following are documents that are important to keep safe. These items establish your
identity, make it easy to locate healthcare and insurance information, and more. They are useful for
times when you might be moving to a new location, going to medical appointments, or accessing

ロ Birth certificate or passport
ロ Driver’s license or state-issued photo ID
ロ Medicare or Medicaid card
ロ Health insurance card/proof of insurance
ロ Power of attorney documents

ロ Certificate of citizenship or permanent resident card (“green card”)
ロ Contact information for family or friends, including phone numbers
Additional Considerations for Residents*
- Newly Long-Term Disabled individuals / or individuals over the age 62 may wish to consider to applying for Social Security Disability SSDI/SSI/SSA if they will not be able to return to gainful employment post discharge or feel that it is the right time for them. ロ Husky Member ID Card and Gray EBT ConneCT Card:
- If your HUSKY Health Member ID card is lost, stolen, or damaged, call Member Engagement Services at 1-800-859-9889 or request a new one through the secure member email.

ロ Proof of Income (Example Annual Benefit Letter):
- You can request one online by using your personal my Social Security account, which will allow you to immediately view, print, and save a copy of the letter.
