Skilled Nursing Facilities and Long-Term Care Facilities

As of July 1, 2022, health care providers who administer immunizations in Connecticut must report electronically to CT WiZ. This includes all state supplied and privately purchased vaccines administered to all patients and employees (for all ages)

If your skilled nursing facility/long-term care facility is not yet reporting vaccinations to CT WiZ, please complete the following steps:

1. Complete the onboarding application

  • There are two ways your facility can electronically report to CT WiZ

  • If your facility has an electronic health record (EHR), you can onboard your EHR so that when you enter into your EHR, immunizations will automatically update patients records in CT WiZ.

  • If your facility does not have an EHR, or has an EHR that is not compatible, you can enter all immunizations administered directly in the patient’s record in the CT WiZ User Interface (UI)

  • In the onboarding application, you will select your reporting option. Once the Department of Public Health (DPH) Immunization Program receives your submitted application online, DPH will reach out to the primary contact with next steps.

 

2. Training on CT WiZ

  • Once your clinic successfully completes onboarding with your EHR, you will be "live” with electronic reporting to CT WiZ and all the immunizations entered into your EHR will be sent to CT WiZ automatically. Your clinic will be compliant with the reporting law.

  • If your clinic will use the UI, DPH will email the primary contact training on how to enter immunizations in CT WiZ, and you will be “live” with electronic reporting to CT WiZ.

3. Request a Username in CT WiZ

  • Any health care provider reporting immunizations to CT WiZ can request access to view their patient’s immunizations records run reports. For information on which type of access to request, please visit our username webpage.
  • You must complete the previous steps before requesting access to CT WiZ

4. Reporting Immunizations in CT WiZ (User Interface/Direct Entry Only)

  • Once your facility has completed training, and all your staff doing data entry have access, you may begin entering immunizations in CT WiZ. Reminder

  • Historical immunizations are any immunizations previously given, at another clinic or yours, that are not currently in CT WiZ prior to when you went “live” with electronic reporting to CT WiZ. See training: Entering Historical Immunizations.

  • Real-time Administered’ immunizations are any immunizations your clinic administered after going live with CT WiZ. This must be reported electronically to CT WiZ within 48 hours of administration. See training: Entering Immunizations Real Time.
