The Connecticut Department of Public Health (DPH) Immunization Program’s Connecticut Immunization Information System (“CT WiZ”) enables real-time, comprehensive immunization information and inventory management to authorized users as permitted by state law.
Pursuant to Connecticut General Statutes § 19a-7h-5, in order to access CT WiZ data, all authorized users must read and electronically sign the CT WiZ Confidentiality Agreement upon initial access and for renewals by the due date prompt in CT WiZ. The renewal is required every two years. CT WiZ Confidentiality Agreements are no longer accepted by email, fax, or mail. As of April 15, 2025, the electronic signature process is required by all who are authorized to access CT WiZ data; this includes those who have direct access and those who report to and query from their electronic health records system. As a reminder, authorized users include the Connecticut Department of Public Health staff/designated contractors, local health directors, health care providers/designated clinic staff, pharmacists, and school nurses.

- Login to CT WiZ:
For a New User for initial access:
For an Existing User signing a renewal:
- Click Yes.
- Review your contact information and add updates. Skip this section if you have no changes.
- Click to review the Agreement on top, Click to Accept, and Click Save.
- Once you hit save, you are done. You are allowed to continue to login and access CT WiZ.
- The date you electronically signed is automatically populated in your user profile in CT WiZ. This date will also enable a reminder prompt for your subsequent two-year renewal to be signed in CT WiZ.
- If you wish to access the Agreement for your records, visit the Laws website for a copy of the Agreement.
Important Note:
Your username access will be inactivated if you do not electronically sign your Agreement by the due date as prompted upon login to CT WiZ.
- If you need support, submit a helpdesk ticket and select:
- System: Immunizations (CT WiZ)
- Category: CT WiZ Login (Username or Password Issues)
- Topic: Existing CT WiZ User-Username or Password Issues or Update CT WiZ user account.
- Once your username request is re-activated by DPH, upon login you will need to Click Yes and electronically sign the Agreement by the due date to continue to access CT WiZ data.
