How do patients opt out of CT WiZ?

If you’re not sure you want a record of which vaccines you’ve received and when you received them, please keep this in mind:

  • CT WiZ is a secure online immunization information system that maintains complete, consolidated vaccination records.
  • Records are kept confidential by law. Only you and healthcare workers can access your immunization records. The information will not be shared with any other people or any other agency unless you sign a Release of Information.
  • You can view, print, and download your or your child’s immunization record when you need it for school, camps, business, travel, and more. Visit CT WiZ: Access My Immunization Record to learn how to view your vaccination record securely online or request a copy.

Below are options on how you can submit your request to opt out of CT WiZ:

Complete this secure online form
Provide your contact information and select "Other/Unsure." In the "Please provide details" box, type that you want to opt out of CT WiZ and include the full legal name, date of birth, and address of yourself or your child (under age 18). You'll need to provide proof of identification.

By Mail or Fax:
Send us a letter requesting to opt out of CT WiZ and include the full legal name, date of birth, and address of yourself or your child (under age 18).

You can call the DPH Immunization Program to request to opt out. You'll need to provide proof of identification.

 

We do NOT accept email, as routine email messages are not a secure way to send patient information.

 

After we receive your request, CT WiZ will no longer update or make available your vaccination record per State law.
