A provider with a current permit to use moderate sedation or deep sedation or general anesthesia may apply to the department for a waiver of the on-site evaluation to administer moderate sedation or general anesthesia at an additional facility location as follows:
Submit an application for waiver online at elicense. To submit your waiver application online, you will need your user ID and password. If you do not recall your user ID or password, you can use the recovery options (Forgot Password?) as well as the 'Register' link in the ‘Access Your Account’ section. If you use the 'Register' link, please disregard the language under 'PLEASE READ' and select 'NEXT' button at the bottom of the page. On the next screen, you will need to match your first name, last name and the last 4 digits of your SSN and create a new password. After creating a new password, please click the ‘Generate E-mail’ button. The system will send you an email with a link to login.
After login, please select ‘License Maintenance’ under ‘More Online Services’ and then ‘Start’ next to your sedation permit.
As part of the application, you will be prompted to enter the facility address and upload the facility’s most recent on-site evaluation form.
After the Department's receipt and review of the application, you will be notified by email of its determination.
