Grant Letter of Support Requests
To request a letter of support from the Department, fill out a letter of support request form and submit this form along with your proposed letter language to DPH.LOS@ct.gov. Staff aim to respond to all requests within 10 working days.
Please be advised that approval or denial of requests for letters of support are at the discretion of CT DPH.
