Facilities Licensing and Investigations Section Strike Monitoring & Contingency Plan

In order to assist facilities in developing a Strike Contingency Plan, the Facility Licensing and Investigations Section (FLIS) has developed this webpage for all strike related resources.

In accordance with state regulations, any licensed facility that receives a strike notice is required to submit a Strike Contingency Plan to the Facility Licensing and Investigations Section (FLIS).

Facilities must submit the plan as soon as possible after receiving the strike notice, but no later than five (5) calendar days prior to the proposed strike date identified in the notice.

All Strike Contingency Plans must be submitted using the standardized format provided by DPH.

For additional information or to access the standardized submission form, please contact FLIS dph.flis@ct.gov or visit the DPH Forms & Applications page.
