Nutrition Programs
- The Senior Nutrition Program provides healthy meals for older adults. It helps reduce hunger and make sure people have enough food. The program also helps people connect with others and supports their health and well-being in the community.
Caregiver Programs
If you are a caregiver, you may be able to get support and services. This can include counseling, support groups, minor home modifications, and even a break to care for yourself. Call 1-800-994-9422 to get started. Caregiver support staff will connect you with the services appropriate to your situation.
Remember, to care for others long-term, we need to care for ourselves, too.

- The Statewide Respite Care Program offers respite services for caregivers. It helps them care for loved ones with Alzheimer's or related dementia.
- The National Family Caregiver Support Program gives caregivers a break. It helps those caring for older adults, disabled grandchildren, or adult children.
- Grandparents as Parents Support offers resources, including support groups and information, to grandparents who are raising their grandchildren.
Alzheimer's Disease and Dementia Resources
- Connecticut's Road Map for Alzheimer's Disease and Related Dementias - A one-pager providing key resources and recommendations.
- Connecticut's Guide for Navigating Dementia Care is a helpful resource for caregivers and care partners. It was created by the state’s Department of Aging and Disability Services, the Department of Public Health, and a team of professionals, volunteers, care partners, and people living with dementia.
Health Programs
- No Wrong Door Initiatives: No wrong Door includes public outreach and coordination with key referral sources, person centered counseling and streamlined eligibility for public assistance. This help supports the goals of each person to live a good life in community.
- Falls Free CT (Falls Prevention): Many older adults (65+) fear falling. They limit their activities and social lives as a result. This can lead to a decline in health, depression, isolation, and feelings of helplessness. This program helps address the issue.
- Live Well, a Chronic Disease Self-Management Program: Live Well teaches skills for managing chronic health conditions. Workshops are available in English or Spanish. They are not a substitute for your physician's care.
