Primary Care Redesign

Overview

In 2022, the Department did an evaluation of our existing primary care programs (PCMH and PCMH+), performed a landscape analysis of what other states and payors are doing in primary care, and formed preliminary recommendations for opportunities for the future of HUSKY primary care.  These actions were taken to align with the Department’s goals to improve the biopsychosocial wellbeing of our members and the Department’s guiding principles to imbed a health equity lens throughout the process.  As we move forward with the evaluation and development of the next phase of primary care, we’ve formed a stakeholder committee, which convenes a diverse group of Connecticut stakeholders bringing a unique and critically valuable perspective in this exciting process.
