Nurse Aide Registration Renewal Requirements
To renew a nurse aide certificate, nurse aides must have worked at least one 8-hour shift of paid employment as a nurse aide or in a position providing nursing related services prior to their nurse aide certificate expiring. Download the Employment Verification Form and complete the nurse aide section. Request that your employer complete the employer section of the form and return it to the nurse aide registry.
The form may be returned to the registry by fax at (860) 707-1983, email to dph.nurseaide@ct.gov or mailed to the address located at the bottom of the employment verification form.
Renewals will be effective two years from the last date of reported/verified employment.
There is no fee for renewal.
Expired/Lapsed Certificates:
If your nurse aide certification has expired and you have been working as a CNA or in a position where you have provided nursing related services, but you did not submit verification of employment, you may submit an employment verification form verifying employment that started prior to the expiration of the certificate.
Renewals will be effective two years from the last date of reported/verified employment.
There is no fee for renewal.
If your nurse aide certification has expired/lapsed and you have not worked as a nurse aide or in a position where you provided nursing related services in a period longer than 24 months (2 years), you are required to retake and pass the Connecticut Nurse's Aide exam (written and practical examinations) administered by Prometric.
