Online registration for CT Transition CoP Meetings is available on the State Education Resource Center (SERC) Events webpage. All participants must register prior to the session. Meetings may be held virtually or in person. Please read the description carefully.
*For Virtual Meetings: Zoom information will be sent one to two hours before your session to the email address entered at registration. Registration links cannot be provided during the session, so please register early!

CT Transition CoP Vision
All students, families, and other key players in the transition planning process will have access to current information necessary to plan for, develop, and implement postsecondary goals and annual transition goals with short-term objectives. These goals will reflect the student’s own voice, values, and vision for life after school, empowering them to take an active role in shaping their future. (Updated 8/21/2025)
CT Transition CoP Mission Statement
The Connecticut Transition Community of Practice (CoP) will bring multiple stakeholders together to collaborate and share ongoing postsecondary transition information, resources, activities, and events. These will be housed in a centralized location that is accessible to all students, families, and transition teams across the state, and ensure that youth voice is included, elevated, and respected throughout the process.
