Production Record Forms

School food authorities (SFAs) may use any production record format that includes the required elements. The CSDE’s production record templates below for the School Breakfast Program (SBP), National School Lunch Program (NSLP), and Afterschool Snack Program (ASP) assist SFAs with including the information needed to meet the USDA’s production record requirements. SFAs may adapt these templates as needed.
