Filing a Required Notice

Connecticut hospitals, hospital systems, and group practices (comprised of 30 or more physicians) are required to submit an Annual Report of information regarding group practices to the Office of Health Strategy (OHS) and the Office of Attorney General (OAG).

The Annual Reports are required to be filed annually by January 15th.

 

Who needs to submit this filing requirement?

  • Connecticut hospitals which include all general, children's, chronic disease, psychiatric, hospital and maternity hospital facilities.
  • Hospital Systems are defined as aparent corporation of one or more hospitals and any entity affiliated with such parent corporation through ownership, governance or membership; or a hospital and any entity affiliated with such hospital through ownership, governance or membership;
  • Group practices that are not owned or affiliated with a hospital or hospital system and comprised of 30 or more physicians.
