In Connecticut, hospitals and health systems are required to report specific financial data related to the fees they charge or bill, particularly for the facility fees charged during the preceding calendar year. The rules governing these filings came into effect on October 1, 2015.

The data must be submitted to the Office of Health Strategy (OHS) annually by July 1 for the preceding calendar year. The facilities must provide data for separate off campus and on campus activity including gross charges, payments, number of visits by payer (Medicare, Medicaid, Private Insurance, and Other) and top 10 procedure codes.

These filings are part of Connecticut's efforts to increase transparency in healthcare pricing, allowing for better oversight and understanding of the charges patients may face for hospital services.

Instructions

Submit required documents 

Submit all required documents by the indicated due dates through the OHS Notifications and Filings portal.

Download the data tables 

By July 1, download the Excel file and complete and submit Table 1 and Table 2.

Complete the Facility Fee Billing Affidavit 

By July 1, all hospitals/health systems must complete and submit this affidavit even if they do not charge facility fees at any on-campus or off-campus hospital-based facility.

Complete the Facility Fee Notification Affidavit 

By July 1, only hospitals/health systems that charge facility fees at any on-campus or off-campus hospital-based facility must complete and submit this affidavit.
 

Sample written notice  

By October 1, facilities must submit a copy of the written notice they provide to patients regarding fees, with tag lines, that are in locations that are readily accessible to and visible by patients. Click the link above to download a sample.

Submit a Billing Statement 

By October 15, facilities must submit a sample of a billing statement. The billing statement should not include any patient identifying information and should also include tag lines.
