Do I need to submit a CON form to replace our MRI, CT, PET or PET-CT.

To change a scanner you already received permission to have:

Go to the CON Forms and Fees page
Download and fill out the CON Replacement form
Upload your form to the Certificate of Need Portal

We will review your form and make sure you had previous permission. Then, we will post a notice on the Certificate of Need Portal and notify you.
