BEFORE YOUR FIRST DAY:
Prior to your first day, we need the following forms completed:
- CT-W4 Connecticut State Tax Withholding Certificate
- W-4 Federal Withholding
- Designation of Retirement Plan Beneficiary (CO-999)
- Designation of Retirement System Tier Plan Beneficiary (CO-931)
- Direct Deposit Authorization and Input Form (CO-1040)
- DSS Notice of Change/Emergency Contact
- Employment Eligibility Verification (Form I-9)
- Employee Information Form
- HEP Enrollment Form (CO-1314)
- Picture Identification, Building Access, and Parking Permit Form
- Retiree Health Fund Enrollment (CO-1300)
- Retiree Health Fund Enrollment (CO-1300B)
- Temporary Situational Telework Application
The above forms can be completed electronically. Please bring all forms with you on your first day.
FIRST DAY
WHAT TO BRING
- Current driver’s license or ID card issued by federal, state or local government
- Date of birth and social security number of anyone you intend to designate as a beneficiary for retirement or insurance purposes
- Birth certificate (long form) for eligible dependents you intend to place on your health and/or dental insurance
- Marriage certificate (if you intend to place your spouse on your health and/or dental insurance)
- Documentation that establishes your identity and employment authorization (current government issued photo identification, social security card, U.S. passport, birth certificate, etc.)
- DD-214 (if served in the military)
PAYROLL - MANDATORY DEDUCTIONS
You will notice deductions to your bi-weekly paycheck as follows:
- Federal Income Tax
- State Income Tax
- Social Security
- Medicare
- Union Dues or Fees, if applicable
- Retirement Deduction (5%)
- Employee Contribution to Retiree Health Care Trust Fund (OPEB) (3%)
UNION DUES/FEES: The unions establish a rate, which will be deducted from your paycheck bi-weekly.
RETIREMENT: All new employees participate in the Connecticut State Employees Retirement System Tier IV plan. Participation in this contributory plan is mandatory at the rate of 5% of your salary. If you have prior service as a state employee, please advise Human Resources for further information.
EMPLOYEE CONTRIBUTION RETIREE HEALTH CARE TRUST FUND: All individuals hired on or after July 1, 2017 must pay a three (3%) contribution for a period of fifteen (15) years or until retirement whichever is sooner, even if they have had periods of prior service.
New employees starting on the first day of a pay period will receive their first paycheck four weeks from the start date.

HEALTH INSURANCE: The State of Connecticut offers two options for health insurance. The options are as follow:
- Anthem Blue Cross and Blue Shield
- United Healthcare (Oxford)
DENTAL: The State of Connecticut offers dental insurance with Cigna.
PRESCRIPTIONS: The State of Connecticut offers prescription drug coverage through Caremark. Prescription benefits are the same no matter which medical plan you choose.
Coverage will begin the first day of the month following your hire date. If you choose coverage, your premiums will be deducted from your bi-weekly pay.

GROUP LIFE INSURANCE: Permanent employees, whether classified or unclassified, full or part-time, will be insured for the basic life insurance plan upon completion of six months of continuous service or nine hundred and fourteen (914) hours of work, whichever is greater, provided a signed enrollment card is on file in the Employee Benefits Unit of the Office of the State Comptroller.
