Value Based Payments
Strengthening Quality – Provider Value Based Payment (VBP) Initiative
To improve Medicaid client health outcomes the Connecticut Home and Community-Based Services (HCBS) American Rescue Plan Act (ARPA) Program aims to create and sustain a value-based fee-for-service delivery model by providing whole-person care through incentive payments to HCBS providers based on clearly defined outcomes.
Funded by ARPA, the initiative will span three years with specific quarterly benchmarks. DSS plans to continue VBP after the initial 3 years with the aim of aligning payment with value. During the initial payment periods, HCBS providers will execute data sharing agreements with Connecticut’s Health Information Exchange (HIE) Connie and participate in the DSS-provided Racial Equity training. HCBS providers who meet designated benchmarks will receive a 1% supplemental payment. The current award period is based on claims paid from November 1, 2022, through February 28, 2023. The timeline below outlines the expectations of participation for Home Health Agencies and Home and Community Based Services in the Value Based Payment Initiative.
For providers who are already participating, please follow guidance below:
To receive the November VBP, providers must complete Requirements 1 and 2 by November 1st.
REQUIREMENT 1:
CONNIE Requirement: To support DSS HCBS ARPA program, Connie is developing a web-based application for HCBS providers to support client care coordination. The purpose of this survey is to help us understand what client information you need to support meeting the program goals.
Providers must participate in the CONNIE survey AND identify a ‘champion’ in the agency who will participate in learning collaboratives and provide input related to DSS’ design and development of initiatives. The CONNIE survey will be distributed via email to the email you have on record with CONNIE. Any changes with designated staff should be reported to CONNIE. The next Value Based Payment Requirement is to complete the following survey by November 1, 2022, and return it to heidi.wilson@conniect.org.

REQUIREMENT 2:
Training Requirement: Trauma Informed Care Racial Equity Training
There are currently four web-based trainings available on the training tab of this website. Two more will be added later this year. Supervisors and managers must complete a benchmark of 80% successful completion of each course in each agency. Non-management staff must reach a benchmark of 25% successfully completing each course in each agency.

For all NEW providers that are interested in participating, the following 2 requirements must be completed.
- Requirement 1: New providers must enter into a data sharing agreement, sign use case, and complete survey with Connie.

To participate, HCBS providers will be required to sign a Simple Data Sharing Organization Agreement with Connie (SDSOA). The formal agreement will be provided through DocuSign for signature. To receive the formal agreement, each interested HCBS provider will complete and submit the online Commitment to Connect form. Despite the form name, submitting this form does not obligate you to participate. Make sure to check to box (3rd question down) that you are interested in the HCBS program. Please note that if your organization does not provide healthcare (e.g. you are not a licensed health care provider in CT), when asked what type of organization, select that you are a community based organization.
