Money Follows the Person is a federal demonstration grant that has been accepting referrals since 2008. The program is funded by the Centers for Medicare and Medicaid Services (CMS) and exists in 48 states. Each state operates under its own operational protocol. The protocol in Connecticut has had many revisions and is currently under review. Below is the most current protocol approved by CMS.
