The DPH Office of Health Equity Draft Strategic Plan (2015 – 2018) was developed by DPH staff in consultation with key partners, and with the technical assistance of CommonHealth ACTION, a national public health organization that aligns people, strategies and resources to generate solutions to health and policy challenges.
This project was funded by the U.S. Department of Health and Human Services’ Office of Minority Health (OMH) within the Office of the Secretary as part of a National Partnership for Action to End Health Disparities capacity-building project with the Association of State and Territorial Health Officials.
General technical assistance and funding administration was provided by the Association of State and Territorial Health Officials (ASTHO).
