There are two versions of the CT Department of Public Health’s Transition to Adulthood survey: A Youth or Young Adult version and a Parent or Caregiver version.

 

If you are a youth or young adult, please choose or fill in the responses that best match your experiences.  If you are a parent or caregiver, please choose or fill in the responses that best describe the experiences of your youth or young adult.

 

The survey is anonymous and confidential, which means that you do not have to worry about anyone linking your name to your answers.

 

The purpose of these surveys is to assess transition experiences of youth with special health care needs in Connecticut.  These survey questions assess key transition issues including: health insurance; doctors and other healthcare providers; resources; health care, school, work, housing, transportation and other transitions.

 

You only need to take this survey once.  If you have already taken this survey, you do not need to take it again.
