Verification of Licenses for Professions other than MD, DO, RN and LPN
Requests for written verification of a Connecticut license are only accepted online. The Department does not complete third party forms. Please be assured that our verifications will not be rejected because we do not complete third-party forms. The licensee will need their user ID and password to login to their account at www.elicense.ct.gov.
If you don’t recall your user ID and password, please select ‘Register’ under ‘Access Your Account’ and enter your first name, last name and the last 4 digits of your SSN. Provided there is a match, the system will display your User ID and email address on file. You will then be able to update your email address and create a new password. If your email address needs to be validated, please click on ‘Generate E-mail’ button. The system will send you an email with a link. Click on that link and login with the user ID and password you entered. You will then be able to login to your account and request the verification.
Once logged in, select 'Request License Verification' at the bottom left portion of the screen. Select the license for which verification is being requested and enter the address where the verification is to be mailed. Please note that if ‘Request License Verification’ link is not visible, you are not logged into your licensure account. In this scenario, you created a new contact in the system separate from your licensure account.
The verification will be sent via U.S. Mail. DPH will not fax, email or send the verification by overnight mail. Once you submit the verification request, you will receive a message on the screen that the verification will be mailed within 3 business days.
If you receive an error message after you enter the address where you want the verification mailed, chances are likely that the error is a result of too many characters entered in the address fields. If you do get this error, please abbreviate the address as best you can,
The verification report is generated from the licensing system and contains the following information: Licensee name; profession type; license number; date license issued; expiration date; license status (active or inactive); basis of licensure (exam, endorsement, etc.) and an indication if there is any past or pending disciplinary history. If past disciplinary action been taken against the license, DPH will include the final order associated with the case.
