How should I report the death of a CTRB member?

Please complete the Reporting the Death of a CTRB Member Form and mail it to this office with a photocopy of the Death Certificate.  Upon receipt of this information, this office will be in a position to contact the proper person(s) in writing with final settlement information.


How should I report the death of a CTRB member's spouse, dependent or co-participant?

Please complete the Reporting the Death of a CTRB Member's Spouse/Civil Union Partner, Dependent or Co-Participant Form and mail it to this office with a photocopy of the Death Certificate.  Upon receipt of this information, this office will be in a position to contact the proper person(s) in writing with final settlement information.
