Primary Care and Community Health Reforms Work Group Overview
The Office of Health Strategy (OHS) formed this work group to align Connecticut around proven capabilities, and flexible model options that support patient-centered, convenient care delivered effectively and efficiently, and supports upstream interventions to prevent conditions from occurring by supporting regional initiatives and collaboratives that address such needs. This group will include discussion of the link between health enhancement communities and primary care. The Primary Care Work Group brings together consumers, providers, public and private payers, advocates, and other experts from across the continuum of healthcare.
In response to Governor Lamont’s Executive Order No. 5, this work group will work with the OHS Cost Growth and Quality Benchmarks and Primary Care Target initiatives’ Technical Team and Stakeholder Advisory Board to set a target within each annual benchmark for increased primary care spending as a percentage of total health care expenditures to reach a target of 10% by calendar year 2025.
