Hospital Community Benefit

APCD Data Request

C.G.S. §19a-127k authorizes the OHS APCD program to release a limited data set for hospitals' community benefit programs.  The limited data set is released to hospitals solely for the purposes of: 

  • Preparing a Community Health Needs Assessment(s); 
  • Preparing and executing implementation strategies; and 
  • Meeting community benefit program reporting requirements. 

Hospitals may request a limited data set through the Community Benefit PortalFirst time users of the portal will be asked to register.
