Connecticut Health Equity Dashboards

The effects of COVID-19 exacerbated health disparities among communities and populations across Connecticut. The Connecticut Health Equity Dashboard is an interactive tool that brings together health outcomes and social and health drivers (such as socioeconomic status, race/ethnicity, housing and transportation) that influence health and well-being. This tool is intended to assist community-based healthcare providers, organizations, and policymakers to assess and mitigate their community's vulnerability risk.

The Connecticut Health Equity Dashboard incorporates measures such as the Social Vulnerability Index (SVI) and the Area Deprivation Index (ADI), offering the ability to evaluate vulnerability risks and health outcomes at both town and subpopulation level in comparison to statewide results. The dashboard is intended to support the development and implementation of targeted interventions to improve health outcomes and reduce health disparities.

The dashboard utilizes 2022-2023 data from sources such as All-Payer Claims Data, Patient Data, the Department of Public Health Vital Statistics Registration Reports and the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS) measures. These data provide insights into access to preventive care, burden of disease, chronic care management and health complications or poor outcomes to help document and address health disparities across the state.
