Greetings from the CT Rural Health Transformation Program (RHTP) at the Connecticut Department of Social Services (DSS)!
Join us on Tuesday, July 21st from 3:00 - 3:30 PM ET for an informational webinar about Connecticut's plans to launch a Program of All-Inclusive Care for the Elderly (PACE) program!
Learn more about what PACE is, how it works, and the steps underway to bring this model of care to Connecticut. Through the RHTP, Connecticut is joining 33 states that already have a PACE program.
What is PACE?
- A home and community-based services program for adults age 55+ who need long-term services and supports
- Designed for people who need a level of care that qualify them nursing home services, but who want to live at home
- All medical and social services are included, paid for by Medicare and/or Medicaid

Rural Health Advisory Council (RHAC)
The RHAC will gather stakeholder input to inform implementation of Connecticut’s Rural Health Transformation plan. It will also support the Department of Social Services by elevating rural perspectives, sharing timely program information, and increasing awareness of RHTP initiatives – all aimed at improving health outcomes and advancing long-term sustainability.
Members will serve in a voluntary advisory role and represent a variety of rural health stakeholders including rural hospitals, federally recognized Tribal Nations, federally qualified health centers, local health districts, behavioral health providers, medical and allied health professionals, community-based organizations, and local businesses.
