Notice of Intent to Renew the 1915(b)(4) Medicaid Waiver for Case Management Services

In accordance with the provisions of section 17b-8 of the Connecticut General Statutes, notice is hereby given that the Commissioner of the Department of Social Services (DSS) intends to submit an application to the Centers for Medicare and Medicaid Services to renew the 1915(b)(4) waiver for case management services for the Connecticut Home Care Program for Elders 1915(c) waiver, the Connecticut Home Care 1915(i) program, and the Personal Care Assistance 1915(c) waiver.  The 1915(b)(4) waiver which is the subject of this renewal application enables DSS to continue utilizing its case management procurement process to assign members of the above-referenced programs to contracted case management providers (access agencies) in accordance with the members’ town and the access agencies’ catchment area.

DSS’ authority to operate under the current 1915(b)(4) waiver expires on 12/31/2025. DSS is not proposing any substantive changes to the 1915(b)(4) waiver at this time.
