Yellow Fever Vaccine Certification and Stamp

Procedures for Healthcare Providers (physician, nurse, pharmacist) licensed in the State of Connecticut to Obtain/Update/Remove a Yellow Fever Vaccine Certification and Stamp.

Connecticut Department of Public Health (DPH) Immunization Program reviews healthcare providers' requests for certification to administer yellow fever vaccine in the state of Connecticut. The DPH will process your requests through our helpdesk and send an email when your certification has been completed.


Procedure for Healthcare Providers to Request to Become Certified to Administer the Yellow Fever Vaccine:

To request to become certified to administer yellow fever vaccine:

  1. Providers must complete the required yellow fever vaccine training prior to certification at: Yellow Fever Vaccine 2023: Information for Health Care Professionals Advising Travelers - CDC TRAIN - an affiliate of the TRAIN Learning Network powered by the Public Health Foundation. CEs are no longer offered but those taking the training can still obtain a completion certificate.
  2. Submit a DPH Helpdesk ticket at https://dph-cthelpdesk.ct.gov/Ticket.
    (select Immunizations (CT WiZ) - Yellow Fever Vaccine Authorization - Request Yellow Fever Authorization)

    In this ticket, please include all the information below:
    • Physician's Name
    • Physician's License & DEA Number
    • Name of Facility
    • Mailing Address of Facility (from where yellow fever vaccine will be distributed)
    • Office Phone
    • Office Fax
    • Office Email
    • Who will receive the yellow fever vaccine: (international traveler, general public, or only for the distributor's clientele or company) please explain:
    • Yellow Fever Training Completed: Yes/No/Need Training Links

  3. Upon certification, the DPH emails the three (3) documents below, and informs Sanofi Pasteur that the provider is now officially certified to administer yellow fever vaccine in the State of Connecticut.
    • Document 1 – An official State of Connecticut certification sent to the provider to Administer the yellow fever vaccine.
    • Document 2 – An official certification sent to Sanofi Pasteur enabling the facility to place their yellow fever vaccine orders. 
      • Please note: All yellow fever vaccine orders are placed directly to Sanofi Pasteur and are not placed at the State of Connecticut.
    • Document 3 –An official certification for the yellow fever stamp, with the physician’s license number on it, which will be used to order the stamp from the Hartford Stamp Company. 
      • Please note: Once this document is received from the DPH, the provider will be responsible to contact the Hartford Stamp Company to purchase a stamp.

  4. Certified yellow fever providers who can administer yellow fever vaccine in the State of Connecticut will be added to the Centers for Disease Control and Prevention (CDC's) Yellow Fever Traveler's Health webpage.
  5. Providers should review the CDC's Yellow Fever FAQ webpage and review the Yellow Fever packet regarding the program once certified.


Procedure for Healthcare Providers to Update or Ask Questions about their Yellow Fever Vaccine Certification:

Providers should submit a DPH Helpdesk ticket at https://dph-cthelpdesk.ct.gov/Ticket.
(select Immunizations (CT WiZ) - Yellow Fever Vaccine Authorization - Questions or Update Information).

In this helpdesk ticket, please include:

  • Physician Name and Facility's Contact Information.
  • The information that needs to be updated or your question.

The DPH will update the information or contact you for follow up if needed.


Procedure for Healthcare Providers to Remove their Yellow Fever Vaccine Certification:

Providers should submit a DPH Helpdesk ticket at https://dph-cthelpdesk.ct.gov/Ticket.
(select Immunizations (CT WiZ) - Yellow Fever Vaccine Authorization - Remove Yellow Fever Authorization).

In this helpdesk ticket, please include:

  • Physician Name and Facility's Contact Information.
  • Date when you will stop providing yellow fever vaccines.

The DPH will contact you for follow up to ensure your facility is removed from the CDC website and your stamp gets returned.
