Who has access to the information in CT WiZ?

Connecticut state law requires that information in CT WiZ is kept confidential and limits who can access it. All CT WiZ users are approved by the Department of Public Health and must sign a user agreement and confidentiality agreement. Here’s who can access CT WiZ:

  • Healthcare providers can access their patients' records
  • Local health directors can access their residents' records for programs to raise immunization rates
  • School nurses can access their students' records

The information will not be shared with any other people or any other agency unless you have signed a release of information and proof of identification and sent it to the Connecticut Department of Public Health.

To submit your release of information to request an immunization record from CT WiZ:

  1. Complete and submit this CT WiZ Release of Information Form, and submit your proof of identification by the Online Submissions Form.
  2. Or if your agency has your own release of information form, you may attach that and your proof of identification to the Online Submissions Form.
  3. Or you may submit a signed release of information and proof of identification by:
    • Mail: CT DPH Immunization Program CT WiZ, 410 Capitol Ave., MS 11 MUN, Hartford, CT 06134, or
    • Fax: (860) 707-1925
    • Do NOT send us your request by email. Routine email messages are not secure.
