Below are common forms that you need to keep on your premises, depending on the type of business you operate.
All Permits
- Age Statement Form: Every permitted premises in Connecticut must keep blank copies of age statement forms available. Keep all completed forms for at least two years. Age statement forms may be kept and stored electronically.
- Invoices and Records of Sales: Every premise must keep a two-year record of their daily sales and wholesaler invoices on their premises unless the Liquor Control Division approved a request to keep your invoices elsewhere.
On-Premises Permits (bars, restaurants, breweries, etc.)
- Coil Cleaning Card: You must have your beer lines cleaned every two weeks and the cleaning noted on a card kept on the permit premises for at least one year from date of last entry.
Off-Premises Permits (package stores, grocery stores, breweries)
- Keg Book: If you sell kegs, you are required to tag the keg and collect deposits. A keg book holds the tags and helps you keep track of the kegs you sell. Contact our License Services division to order a keg book.
