Abandonment Affidavit: Used if there was a prior permit at business location but no alcohol was purchased from prior permittee
Add Live Entertainment: Add entertainment endorsements (bands, acoustic music, comedians, magicians, etc.) to your permit after filing your application (you may select live entertainment options on the first page of your application)
Age Statement Form: Required to be kept on premises at all times; used when age of patron in question
Affidavit of Seller’s Unpaid Obligations: Used if your business bought alcohol from the prior permittee
Authorization to Release of Financial Information/Personal History*: Must be filled out by permittee and each member of the backer to authorize a background check
Authorization for Release of Backer Legal Entity Info: Must be signed by a member of backer; allows DCP to obtain financial information if required.
Backer Amendment: Only use if directed by Liquor Control; updates your backer members before issuance of final permit.
Backer’s Financial Statement*: Established that your business has the funds to cover initial and anticipated costs and expenses; use only if directed by Liquor Control to correct mistakes (information in this form is embedded in online application)
Boat or Livery Vehicle Add-on: Used to add a boat or vehicle to a transporter's permit.
CHRO Criminal Conviction Worksheet: Necessary if permittee or member of the backer has a felony history
Coil Cleaning Card: Keep next to tap system to record beer line cleanings every two weeks.
Consultant/Attorney Representation Authorization: Used if you have an attorney, consultant, or other third-party helping you fill out the application.
Corporate Officer & Board Member Change Form: Update your officers and board members when no ownership interest changed.
Corporate Structure Change Form: Change your corporate structure when no ownership interest changed (e.g., convert from an LLP to a Corp.)
Disclaimer Statement: Used if a person provided funds to open a liquor-permitted premises but has no ownership interest
Farm Winery Crop Loss Report: Documents when farm wineries lose crops and need adjust percentages
Farm Winery Retail Location Form: Local approval for additional farm winery retail locations
Farmers' Market Additional Location Form: Used by manufacturers to add new farmer's markets to their farmers market permit
Fire Marshal Inspection Form: Documents inspection by local fire marshal and that premises may be operated under fire code; fire marshal may sign front page of application instead of submitting this form.
Grocery Beer-Breakdown of Sales: Used to determine if grocery store meets threshold requirement of food sales.
Hotel Guest Bar(s): Add hotel rooms to hotel permit
Keg Registration Form: Request new keg books for selling kegs to consumers
Patio/Ext. of Use/Additional Consumer Bar: Add a patio, additional space, or more bars to your premises
Permittee Amendment: Only use if directed by Liquor Control; adjusts your permittee before final permit approval
Provisional Permit Form: Submitted if you want a provisional permit after submitting your online application
Public Health Certificate: Must be signed by local health official and states that your business is authorized to sell food; applicant can submit a copy of the food service license instead
(1) Removal, (2) Stipulated Agreement for Removal (Hardship): These forms are used if you want to move your business and permit to a new location. Use hardship form if factors beyond permittee/backer's control require relocation.
Substitute Permittee: Used if you want to change your current permittee
Transfer of Interest / Stock: Updates the owners of the backer entity due to sale, gift, or other transfer
