This registration permits practitioners to distribute, dispense, conduct research, administer, or procure controlled substances in the course of their professional practice as permitted by the Department of Public Health or other governing agency.
Practitioners include but are not limited to:
- Medical Doctors (MD)
- Osteopaths (DO)
- Dentists (DDS, DMD)
- Veterinarians (DVM)
- Advanced Practice Registered Nurses (APRN)
- Physician Assistants (PA)
- Scientific Investigators
- Midwives with Prescriptive Authority
- Podiatrists (DPM)
- Optometrists
Prerequisites
An individual applying for this registration MUST have a valid active registration with the Department of Public Health.
Required Documentation
No additional documentation is required unless you have been convicted of a felony or have had your license surrendered, revoked, suspended, limited, denied or if any such action is pending.
Application
Please Note: This is a mandatory requirement for licensed medical personnel who prescribe controlled substances in Connecticut.
To apply for this registration you must first have an application on file with the Department of Public Health. When registering, be sure to use the same log-in information for the application that you submitted to the Department of Public Health or that was emailed to you by the Department of Public Health.

Renewal
All registrations expire biennially on February 28th of every odd-numbered year.

Reinstatement
A registration may be reinstated provided this reinstatement form and the applicable fee are submitted no later than two years after the date of expiration or you must reapply. To reinstate an expired registration click here.
Other Important Information
- Drug Enforcement Administration (DEA) registration will be required after obtaining the Controlled Substance Registration for Practitioners. The application can be found here: www.deadiversion.usdoj.gov
- Registration with the Connecticut Prescription Monitoring and Reporting System (CPMRS) is required for all practitioners with a Controlled Substance for Practitioners Registration. For more information and to register, visit the Prescription Monitoring Program website.
