What are my responsibilities as a Mandated Reporter?

This portal was created for the purposes of giving birthing hospitals the ability to file online reports (DCF- 136) of abuse or neglect to the Department of Children and Families OR to create a CAPTA Notification for those newborns identified as substance exposed and consistent with the criteria associated with a notification.  Note that this website is only for referrals involving newborn children and hospital staff, any other child protective services related referral should be made by calling the DCF CARELINE at 1-800-842-2288. During the online submission process you will be asked specific questions that will help guide your filing to the most appropriate pathway.

The Department of Children and Families, in collaboration with the CT Data Collaborative and in partnership with DMHAS and CHA, created an interactive dashboard to make CAPTA data more accessible to community partners and the public. The dashboard allows hospitals and community members to explore a wide range of indicators by hospital, including total notifications, substance exposure, demographics, family care plans, child abuse reports, details on education, services, and referrals, and more. Users can easily navigate from a statewide view of data to a more tailored view both by year and by hospital. Tabs located at the top of the page indicate data points (Summary Data, Family Care Plan Detail Report) that can be explored both individually and comparatively by selecting a year and hospital, or on a statewide level by year. Facilities with less than 100 notifications are combined into one category to ensure privacy.  This dashboard will be updated annually, and currently displays information for the years 2019-2024.  Please use this link to navigate to the dashboard.
