Out-of-State Registration Form

A qualifying out-of-state patient and/or caregiver must complete the registration form below, or its equivalent, each time before medical cannabis may be purchased.

In addition to completing the required form, a qualifying out-of-state patient and/or caregiver must present the following to a hybrid retailer or dispensary facility:

• A valid qualifying credential from the other state; and,
• A valid driver's license or other government-issued photo identification (must show residency in the same state as the valid qualifying credential).

The registration form, along with copies of the qualifying patient and/or caregiver’s ID and valid qualifying credential, must be retained by the hybrid retailer or dispensary facility for no less than three years.
