As a physician, physician assistant or advanced practice registered nurse, how can I get a copy of the form that allows me to certify my qualifying patients for the palliative use of marijuana?

The Department has developed an online registration process so that you can certify your patients electronically. You can access the system here: https://www.biznet.ct.gov/dcp-mmrp

Before certifying patients for marijuana, you will be required to register with the Connecticut Prescription Monitoring and Reporting System (CPMRS) so that you can have a complete picture of your patient’s controlled substance use, including prescriptions by other providers. If you are not already registered with the CPMRS, you can do so at any time by following the instructions here.
