Notices of Material Change

Notices of Material Change.  Group practice combinations that would result in a material change (generally resulting in a group practice of eight or more) must file the Notice of Material Change with the Office of the Attorney General (“OAG”) (and not the Office of Health Strategy (“OHS”)) at least 30 days prior to the effective date of the material change, as required by CGS 19a-486i (c).  The form should be emailed to AG.PhysicianAcquisitions@ct.gov and may be accessed at Notice of Physician Acquisition (ct.gov)

Annual Group Practice Filing

Group Practices - Hospitals, hospital systems and group medical practices comprised of thirty or more physicians must complete the Group Practice Form, available on the OHS page Group Practices.The form must be submitted to the OAG and OHS by uploading the form to the OHS Notifications and Filings portal, as provided on the OHS page Group Practices  by January 15, annually, for the prior year.

Annual Hospital Affiliations Filing

Affiliations Filing - Connecticut hospitals and hospital systems must submit a written report describing each affiliation with another hospital or hospital system to the Office of the OAG and OHS.The Annual Report using the Affiliation Filing Form must be filed with OAG and OHS by uploading the form to the OHS Notifications and Filings portal, as provided on the OHS page Affiliate Filings (ct.gov) no later than January 15, annually, for the prior year.See Affiliate Filings (ct.gov)
