  The Asthma Action Plan (AAP) is a written plan that one develops with their doctor to help control asthma. This includes a list of all medications with instructions with a focus on achieving long-term control and asthma management and a plan of action for when asthma symptoms arise.

• Chronic Disease Prevention Plan

  The Department of Public Health (DPH) released Live Healthy Connecticut, a coordinated chronic disease prevention and health promotion plan, which focuses on creating and maintaining health equity in the areas of nutrition, physical activity, obesity and tobacco use.

• Connecticut Immunization Program

  The Connecticut Immunization Program's mission is to prevent disease, disability and death from vaccine-preventable diseases in infants, children, adolescents and adults. The program uses surveillance, case investigation and control, monitoring of immunization levels, provision of vaccine, and professional and public education.

• Connecticut Mosquito Management Program

  The Connecticut Mosquito Management Program, a collaborative effort of five state agencies, is responsible for monitoring and managing the state's mosquito population to reduce the potential public health threat of mosquito-borne diseases. It uses a combination of education, surveillance, source reduction, larval and adult mosquito control and personal protective measures.

• Diabetes Prevention and Control Program (DPCP)

  The Diabetes Prevention and Control Program (DPCP) is designed to provide citizens with resources promoting diabetes awareness, prevention and information. The DPCP works with public health systems in communities to examine diabetes issues statewide.

• Heart Disease and Stroke Prevention Program (HDSP)

  The Heart Disease and Stroke Prevention Program (HDSP) seeks to educate citizens about heart disease and stroke through public health strategies, preventative information and resources. The focus of the program is on lifestyle and providing citizens and communities access to early and affordable detection and treatment of cardiovascular disease.

• Injury Prevention Program

  The mission of the Connecticut Department of Public Health's (DPH) Injury Prevention Program is to promote a safe and healthy Connecticut by reducing factors associated with intentional (e.g. homicide and suicide), unintentional (e.g. falls and motor vehicle accidents) and occupational injury.

• Lead Poisoning Prevention and Control Program

  Lead harms children's nervous systems and is associated with reduced IQ, behavioral problems and learning disabilities. In large doses, it can cause coma, convulsions and death. The Lead Poisoning Prevention and Control Program seeks to prevent and educate the public about lead poisoning, specifically in children.

• “Live Well” Chronic Disease Self-Management Program (CDSMP)

  The State Department on Aging coordinates the "Live Well", Chronic Disease Self-Management Program (CDSMP) and holds workshops at various locations throughout the state. Live Well is targeted to older adults with chronic health conditions and helps them learn practical ways to deal with issues such as pain and fatigue, nutrition, exercise and talking with your doctor.
