Overview of HFC Documentation Forms

All public schools that have chosen to implement HFC under Section 10-215f of the Connecticut General Statutes must submit the online HFC Compliance Form to the Connecticut State Department of Education (CSDE) by November 30 of each year. In addition, selected school districts must also submit lists of foods and beverages sold to students from all sources.



Update for School Year 2022-23

For school year 2022-23, the CSDE modified the HFC documentation process due to the ongoing COVID-19 public health emergency. The established procedure for HFC documentation for this school year (July 1, 2022, through June 30, 2023) is as follows:

For eligible local educational agencies (LEAs) in good standing with HFC, the CSDE will accept the LEA's approved local board of education or governing authority resolution language certifying compliance with the Connecticut Nutrition Standards (previously submitted to the CSDE by July 1, 2022, as required by Operational Memorandum No. 05-22) as HFC documentation for school year 2022-23. Participating LEAs must still meet HFC requirements and maintain HFC compliance documentation but are not required to submit additional documentation to the CSDE for the current school year.
