Summary Description

This document presents the data reporting requirements and documentation for insurers participating in the Robert Wood Johnson Foundation (RWJF) Partnership for Long-Term Care.  These data requirements (referred to as the “Uniform Data Set”) have been developed collaboratively among the four states participating in the RWJF program.

The UDS was negotiated by the four participating State programs in an effort to accommodate insurers who may wish to participate in more than one state.  The State programs are:

• California Partnership for Long-Term Care

• Connecticut Partnership for Long-Term Care

• Indiana Long Term Care Program

• New York State Long-Term Care Insurance Project
