Supporting Arts Grant Program

Final Report

 

FY23 grantees are responsible for submitting a final report no later than October 30, 2023. 


Note:  If your organization is awarded FY24 grants funds, these funds will not be released until the FY23 final report is submitted.


Instructions:

  1. Log in to your SurveyMonkey Apply account at: https://ctofficeofthearts.smapply.org

  2. Look for the “box” for your FY23 Supporting Arts grant application and within that box look for the green “Start” button.

  3. Click the green “Start” button.  This will open a page that will display “Your Tasks” for the final report. 

Once you complete all tasks, be sure to submit your final report.  The system will send a confirmation email if you successfully submitted the final report.  Be sure to check your spam/junk/other in-boxes for this email.  It comes from Connecticut Office of the Arts / SurveyMonkey Apply On-line Platform (noreply@mail.smapply.net).
