Final Report
FY23 grantees are responsible for submitting a final report no later than October 30, 2023.
Note: If your organization is awarded FY24 grants funds, these funds will not be released until the FY23 final report is submitted.
Instructions:
- Log in to your SurveyMonkey Apply account at: https://ctofficeofthearts.smapply.org
- Look for the “box” for your FY23 Supporting Arts grant application and within that box look for the green “Start” button.
- Click the green “Start” button. This will open a page that will display “Your Tasks” for the final report.
Once you complete all tasks, be sure to submit your final report. The system will send a confirmation email if you successfully submitted the final report. Be sure to check your spam/junk/other in-boxes for this email. It comes from Connecticut Office of the Arts / SurveyMonkey Apply On-line Platform (noreply@mail.smapply.net).
